Complete Patient health form
Download the Patient Registration Form fill it out, and email it to us prior to your visit.

Arrive for your appointment
A welcoming space, caring doctors, and personalized attention — everything your family deserves in pediatric care.
What to bring:
- Insurance card and parent/guardian photo ID
- Completed patient forms
- Your child’s immunization and medical records
- A list of current medications, vitamins, or allergies
- Any records from your child’s previous pediatrician
Before your visit:
- Please arrive 15 minutes early for your first
appointment
- Allow extra time for parking and locating our office
- Bring any questions or concerns you’d like to
discuss with Dr. Desai and the team
